To Register with Good Medicine and make your first appointment the “Complete New Patient Registration Packet” will need to be downloaded and filled out completely. These must be mailed or faxed to office prior to appointment being made. Other forms listed here may be needed by established patients scheduling their first one hour Good Medicine Consult.
